Texas Patient Rights & Records Request Instructions

This notice explains how to request your medical records, how to contact our licensing authority, and how to file a complaint.

 

This information is provided in accordance with Texas Health & Safety Code §181.105.

How to Request Your Medical Records

 

You have the right to request a copy of your medical records.

 

Step 1: Submit a written request
You may request your records by:

Step 2: Include the following information

  • Full name

  • Date of birth

  • Description of the records requested

  • Delivery preference (email, mail, or in-office pickup)

  • Signature

Step 3: Identity verification
We may contact you to verify your identity before releasing records.

Step 4: Processing time
Requests are typically processed within 15 business days.

Step 5: Fees (if applicable)
A reasonable fee may be charged for copying or preparing records in accordance with Texas law.
